On 04-07-16, an employee was adjusting/operating a coil machine when two looping blocks came together and pinched the employee's finger, amputating the left index fingertip.

On May 8, 2015, an employee was putting an additional bend into a double torsion spring at a large servio table (wire bender) machine. He put his left hand on top of the part, and his left finger got caught between the bending mechanism (steel block) and the stationary block. He suffered a left ring finger amputation.

Barber Spring Company

An employee was approaching the exit end of an automated and fully enclosed shot machine. A spring that was discharging jammed between the discharge pusher and the discharge transfer plate. The spring unjammed suddenly. Due to the excess force created between the discharge pusher and the discharge transfer plate, the spring shot out of the machine to the stop plate, a distance of approximately 8 feet. At that exact moment, the employee's hand was near the discharge table and the spring struck the employee's right arm, catching it between the spring and the stop plate. The employee suffered broken bones to the lower right arm (wrist area) as well as severed tendons and nerves.

On March 27, 2018, an employee was operating a mandrel machine when it amputated the employee's right hand/arm.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
