During disassembly of a spent perforating-gun tool string, an employee attempted to carry the setting tool (designed to set a plug in the well) section from the external stands into the trailer for breakdown. When he lifted the setting tool in the vertical position, he placed his right hand over and into the open end of the sleeve. At that point, the internal mandrel shifted further up to the open end. His right middle fingertip was pinched by the mandrel and the set-screw opening at the end of the sleeve, amputating the distal phalanx.

An employee climbed the steps of a work platform to inspect the passenger side of a pump trailer that was leaking. As he was descending from the platform, he slipped and fell into a sitting position on top of a bracket that went through his coveralls and punctured the inside of his right thigh.

An employee injured his fingertip while rebuilding a down-hole tube. He was taken to the hospital where part of his finger was amputated.

The injured employee was holding a board that was being cut by another employee. The board kicked back, causing the saw to amputate the employee's left little finger and lacerate their left hand, resulting in nerve damage.

On September 26, 2023, an employee was working in a trench box while a co-worker was lowering a piece of plastic pipe by hand using a rope. The pipe swung and struck the employee's hand, fracturing their right thumb and middle and ring fingers. The employee was hospitalized.

An employee went over to a co-worker's station while the co-worker was using a powered meat knife. As the injured employee was pointing, his right index fingertip was amputated.

Two employees were using a handheld banding tool to make banding tape for tent set-ups. One employee was holding the banding tool and the injured employee was looping a metal clip around the tape. The tool closed while the injured employee's hand was near the banding mechanism. The banding tape caught the employee's right ring finger as it tightened, resulting in a partial amputation.

An employee was holding a branch against a log on the ground to make a wedge while another employee was using a chainsaw to cut the wedge. The saw slipped and struck the employee holding the branch, resulting in a laceration to the right wrist.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.

An employee was using a torch to loosen a bolt on a water trailer. Natural gas was present in the water and it ignited, causing a fire. The employee sustained third-degree burns to their upper leg.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
