An employee was under a conveyor belt, troubleshooting a vibrating motor. The employee tried to get up, and the conveyor belt contacted the employee's shirt and pulled the employee's left arm into the rollers of the belt. The arm was broken.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.
