On May 31, 2024, an employee was at a facility getting the propane tanks on the catering truck filled. The tanks overfilled and exploded causing second-degree burns to the employee's face, ear, abdomen, legs, and feet.

An employee was power washing a metal tank from an elevated platform. They fell through a gap between the platform and the tank. The employee landed on the ground and sustained fractures to their ankles and feet.

An employee was installing a metal roof when he stepped on a skylight. The employee fell through the skylight to the ground, resulting in head and shoulder fractures.

While he was in the attic space above the garage, the wood he was walking on broke, causing the employee to fall through the sheetrock to the concrete floor below. The employee bumped his head and sustained a shoulder injury as well as a broken ankle.

An employee was putting a tarp on a water tank while standing on a trailer. She lost her footing and fell through the trailer bed, striking the corner of the trailer bed. The employee sustained internal bleeding.

He sustained injuries to his head, torso, and shoulders. A guardrail was in place at the time of the injury.

A photographer was walking through a studio toward the production table during a photoshoot. The photographer tripped on a power cord that was taped in place on the floor with a cord cover. The photographer fell, fractured her right hip, and was hospitalized.

An employee was on a temporary loading dock, receiving pieces of a large tent structure from a telehandler. She was working to straighten a packing blanket when the telehandler lowered it's tines onto her left hand. Her middle finger was degloved from the first knuckle up. The finger required surgical amputation from the first knuckle up.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
