An employee had completed a butt-weld on two wire coils and had tossed the welding cable behind him. He was then on his way out of the work area when he stepped on either the cable or the connector to the cable. The cable then rolled and he fell onto his right side. He was unable to move his right leg due to a fractured right hip.
An employee was performing a butt weld on two metal rods. He lined up the metal rods in the clamp and did not pull his hand out of the way when the clamp took hold and the butt weld was activated. His left index finger was partially degloved and amputated.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
