An employee was about 25 feet up a tree, trying to cut off a large overhanging tree limb. The limb he had tied off to broke. He fell to the ground, landing flat on his stomach. He suffered collapsed lungs, fractured ribs, and an internally relocated stomach, requiring surgery.

An employee was working on slip forms from a suspended scaffold. The employee jumped from the suspended scaffold to the ground about 25 feet below. The employee's right hip was injured.
