On February 8, 2024, an employee was using a ladder to reach a roof to repair a hole. The employee fell from the ladder to the ground, suffered a back injury, and was hospitalized.

On November 4, 2021, an employee attempted to unbolt a lamp post that was attached to a pulley system and tethered to a forklift. While the load was suspended, the employee reached under the lamp post to remove a nut and bolt. The load shifted and the lamp post landed on the employee's hand crushing their fingers and resulting in a partial finger amputation.

On November 25, 2020, an employee was trying to reach an upper conveyor belt to fix a jam. The employee's foot contacted the moving lower conveyor belt and his leg was pulled between a metal support bar for the conveyor and the chain of the conveyor belt resulting in a broken leg.

An employee was climbing a fixed ladder to access an overhead crane to perform work. The crane was still energized. When the employee touched the crane, it shocked him, causing him to fall to the ground below. He suffered a fractured back and collapsed lung.

On August 8, 2017, an employee was operating a tugger vehicle when the employee's left foot was caught and fractured between a post and the cart.

An employee was walking in an aisle when a forklift backed up over her foot, resulting in a skin injury that required surgery to repair.

An employee was looking into the bucket of a front-end loader to see how much material was in it. A skid steer backed into the employee, who then fell into the edge of the front loader's bucket and suffered internal bleeding in the abdominal area.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
