An employee was using a manual lathe to polish and deburr a sheave shaft with emery paper. The rotating shaft pulled in the employee's hands, injuring the employee's hands and arms.

An employee was installing a lock out latch on a knife blade circuit. The employee contacted the energized top contacts in the electrical box that resulted in an arc flash. The power source was not locked out at the time. The employee sustained second and third degree burns on the front of his upper body, face and hands.

An employee was walking when they tripped. They extended their right hand to catch themselves and came in contact with a fan blade, resulting in an amputation to their right middle fingertip.

An employee had just completed a visual inspection of a dust collection system. As he turned to leave the area, he stumbled and reached out to catch himself. His arm then landed near the outfeed roller conveyor of the planer machine. The employee sustained a laceration to his right forearm with the skin pulled back that required stitches and a skin graft. The employee also sustained a forehead laceration.

At 2:25 p.m. on January 24, 2022, an employee lost their footing while walking down steps after delivering mail. The employee's left ankle was broken.

An employee was loading a nozzle, weighing approximately 500 pounds, onto a positioner. The stainless steel nozzle slipped off the positioner and struck the employee's right leg. The employee's right tibia was fractured and their left knee was bruised.

An employee was preparing a hammer and dies for operation. The ram prop was inserted in the hammer to secure the ram. While the employee went to apply grease to the die shim in the hammer, the ram moved downward. The motion dislodged the prop, causing the ram to drop and crush the employee's right hand and forearm. The employee sustained amputations to their hand and arm and was hospitalized.

An employee was doing a tool changeover on a table mill and changing the mandrel. His hands were on both wrenches with his right thumb was between the two wrenches as he worked to unlock the mandrel. The wrenches came together, crushing his thumb. The employee was hospitalized with a partial amputation of the thumb.

TSK Partners

An employee was helping to remove tooling that was stuck in a forging press (a full-revolution mechanical power press). The press cycled, resulting in partial amputations to two fingers on his left hand.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
