An employee was acting as a fire watch during operations to weld off steel beams when a steel beam came loose and fell onto his left hand, resulting in partial amputations to four fingers (up to first knuckle) and lacerations to the wrist, face, and ear.

An employee was on an extension ladder installing a panel and caulking units. The employee fell from the ladder onto the concrete pavement below. The ladder appeared to have struck the employee as it also fell. The employee lost consciousness and was hospitalized for a head laceration requiring sutures, a concussion, and broken ribs.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
