On March 5, 2025, an employee was performing a post-rain assessment when they located and picked up a lead. The employee received an electrical shock to their left hand and shoulder.

An employee was hanging a sign while standing on the first step of a trailer. The employee lost their footing and fell to the ground, resulting in a hip injury.

On January 14, 2021, an employee was moving toward a hallway junction to re-position an exit sign post when he turned around and tripped, falling to the ground. The employee suffered a dislocated shoulder and fractured right hip that required hospitalization.

An employee was working on a beam about 12 feet from ground level, prepping for welding operations. He became lightheaded and dizzy due to the heat and fell from the beam. His personal fall arrest system stopped him from falling to the ground, but his head struck the beam. He lost consciousness and was hospitalized.

An employee was helping to remove a manhole cover when the tool used to remove the manhole cover slipped out of the groove, causing the heavy cover to crash down on the employee's finger. The employee suffered a left middle finger laceration that resulted in a fingertip amputation.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

An employee was fixing a broken purlin at a worksite when they fell from the framing to the concrete floor approximately 19 feet below. The employee sustained brain bleeding and fractures to their face, vertebrae, and wrists.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.

An employee was unloading a forklift from a trailer to the ground. The employee was climbing down the ladder of the trailer when their foot slipped through one of the rungs, causing them to fall backward onto the forklift forks. The employee was hospitalized for fractured ribs.
