An employee was walking toward the back door to unload a transmission when they slipped in a puddle of water on the concrete floor. The employee fell to the floor and sustained a fractured hip.

An employee was turning a brake rotor and went to adjust the cutter teeth of the brake lathe when his gloved left middle finger was pulled into the cutting tooth. The employee sustained a fingertip amputation halfway through the nail.

On January 4, 2021, an employee was walking in the parking lot when they slipped on black ice and hit their head, resulting in a concussion that required hospitalization.

An employee was outside the back of the store, carrying a plastic tote of parts for delivery, when he tripped and fell down a flight of eight stairs, striking his head on the pavement. He was found unconscious at the base of the stairs. He suffered a head laceration and cranial fracture.

On August 11, 2018, at approximately 7:30 a.m., an employee was stocking product. He was holding an alternator part in his left hand and reaching for a part with his right arm when he suffered a right shoulder fracture.

An employee was driving a cart around the driving range to pick up golf balls. The vehicle struck an in-ground target causing the employee to strike his forehead on the windshield. The employee was hospitalized with fractures to their nose and cheekbone and lacerations to their head and hands.

An employee was operating a picker when it struck a stack of pallets. His body moved forward and he struck his face on the computer monitor in the vehicle, resulting in a concussion, laceration, and blurry vision.

An employee was driving a lift in reverse through an aisle while the lift was loaded with a headboard. The headboard caught merchandise in the aisle racks, causing it to shift and contact the employee's right leg. The employee suffered a broken leg.

An employee was driving a utility task vehicle (UTV) through the parking lot to the boat yard to meet a co-worker. The UTV and slid into a curb, causing it to flip on its side and land on the employee's leg, resulting in a broken leg.

On December 7, 2023, an employee was driving an order picker down an aisle when the mast of the vehicle struck a material pan that was hanging out of the rack. The pan fell and struck the employee, resulting in rib fractures and a spleen laceration.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
