An employee was troubleshooting a process line. The metering valve was removed, but residual pneumatic energy was still present in the supply line. The employee was dislodging debris in the butterfly valve. Once the debris was dislodged, the valve closed, catching the employee's left middle finger and causing a fingertip amputation.

An employee was painting a motor. His right hand became entangled by moving parts in the gearbox, and he suffered amputations to the middle, ring, and little fingers.

An employee was changing out a motor. While it was being removed, the motor shifted and pinched fingertips on the employee's left hand. The employee suffered partial amputations to the last joints of the ring and middle fingers.

Two employees were traveling in a utility vehicle. As the driver made a sharp turn, the vehicle overturned and caught the passenger's right leg between the vehicle and the ground. The employee's right fibula was fractured and lacerated.

An employee responded to a call about issues in the silo. The blower motor amputated the employee's right index and middle fingers, and fractured the employee's ring finger. The employee underwent surgery.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

Employees were at the trucks to leave for lunch. The injured employee was putting the cone back after the crew leader pulled the truck forward from the designated worker parking area. The injured employee then ran back to the truck. As he went to get in the truck, he said the upper thigh area of his leg was hurting. He laid on the ground outside the truck and crew leader could see a bump/bulge in the the injured employee's upper thigh area below his hip. The employee sustained a fractured femur and required surgery.

An employee was helping a contractor unload a delivery of two new dock plates from a trailer onto a forklift. The employee s left index and middle fingers were pinched by the dock plates, resulting in amputations to both fingers above the first joint.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was moving wet bundles of shingles on a truck bed, when one of the bundles slipped out of his hands. He tripped over a pallet, fell off the bed of the truck, and landed on his left side. The employee sustained fractures to three ribs, and a displaced iliac fracture.

On August 12, 2025, an employee was loading shingles onto the roof of a new-construction home when be began to suffer from heat exhaustion and dehydration.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
