An employee was removing a small section of exterior wall assembly above a precast concrete panel that was being removed. While he was removing debris around the top of the precast concrete panel, the precast concrete panel shifted, causing his left middle finger to be caught between the top of the precast concrete panel and the bottom of a horizontal structural steel member above. His left middle finger had to be medically amputated.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
