An employee tripped over an employee's feet and struck a locker door, injuring her head.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was loading a glue drum and pushing the glue drum into the loader while the drum was still elevated on the cart. The employee's hand was caught between the drum and the loader and their fingertip was pinched by the lid of the glue drum, resulting in amputation to the fingertip.
An employee was cleaning debris off the floor using a cleaning cloth. As the employee was on the floor wiping the debris, the commercial washing machine was started and the injured employee was pinned between the spoke and the frame of the machine, resulting in a fractured rib.
