An employee was welding a vessel and put his hand down in a pre-cut hole to roll the vessel. His hand was then caught in a pinch point between the wheel roller and vessel, severely cutting his left index finger, which had to be surgically amputated at the distal interphalangeal joint.
A temporary employee was moving a metal cabinet when his left pinky finger was smashed between the cabinet and a forklift tine. His finger had to be amputated.
