As an employee crossed the door threshold into the building, the employee's left foot slipped and caught a rug edge, causing the employee to fall onto the floor. The employee was hospitalized with injuries to the left knee and left eye, as well as a left arm fracture.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
On March 19, 2025, at 1:35 p.m., two employees were loading a tractor trailer that was backed into a dock and connected to the dock lift. The driver began to drive away while the employees were still on the dock lift, which then collapsed.
