An employee was climbing down a ladder when it slipped. The employee fell, suffering a broken scapula and broken ribs. The employee was hospitalized.

An employee was emptying a hopper into an outdoor roll-off container. The employee exited the forklift to dump the hopper when it tilted forward and their left hand was caught between the right fork and the bottom framework of the hopper. The employee sustained partial amputations to their index and middle fingers.

An employee was troubleshooting a belt and pulley system on a furnace. As the employee checked the tension on the belt, their fingers were pulled into the pulley, resulting in amputation of three fingertips. The belt and pulley system was not locked out at the time.

An employee's right glove was caught between the roller, belt, and conveyor under a powered belt conveyor. The employee's arm was broken.

An employee was disconnecting a hose at the unloading bay to move a vehicle to another location. The valve was not shut off at the time and hot asphalt was released, burning the top of both of the employee's hands and wrists. The employee was hospitalized.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

Employees were at the trucks to leave for lunch. The injured employee was putting the cone back after the crew leader pulled the truck forward from the designated worker parking area. The injured employee then ran back to the truck. As he went to get in the truck, he said the upper thigh area of his leg was hurting. He laid on the ground outside the truck and crew leader could see a bump/bulge in the the injured employee's upper thigh area below his hip. The employee sustained a fractured femur and required surgery.

An employee was helping a contractor unload a delivery of two new dock plates from a trailer onto a forklift. The employee s left index and middle fingers were pinched by the dock plates, resulting in amputations to both fingers above the first joint.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was moving wet bundles of shingles on a truck bed, when one of the bundles slipped out of his hands. He tripped over a pallet, fell off the bed of the truck, and landed on his left side. The employee sustained fractures to three ribs, and a displaced iliac fracture.

On August 12, 2025, an employee was loading shingles onto the roof of a new-construction home when be began to suffer from heat exhaustion and dehydration.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
