An employee was operating an electric pallet jack and turned toward a stationary object. The employee jumped from the pallet jack in order to avoid a collision and their right foot was caught between the pallet jack and a parking block.

An employee was reversing an electric pallet jack into an empty wooden pallet when he slipped off the pallet jack and his left foot was run over resulting in a partial amputation of his big toe.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was making a delivery in a truck. As he exited the truck, he fell on a step and then to the ground, resulting in a fractured tibia.

An employee was picking items from the freezer section of the warehouse. When he stepped down onto the floor, he turned on the planted foot, and felt a "pop" and pain in his knee. The employee was hospitalized with a dislocated knee and two torn tendons. He required surgery to repair the tendons.

At 7:40 a.m. on October 3, 2025, an employee was unloading a bulkhead at a dock door. As he pulled the bulkhead out of a trailer, its plastic lip caught on the dock plate, causing the employee to fall and twist his left leg, knee, and hip. The hip was broken and he was hospitalized, requiring surgery.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
