An employee was participating in pallet jack training. As the employee was traveling backward, the vehicle collided with a metal beam resulting in a contusion and laceration to his left leg.
An employee shrink wrapped a pallet of selected cases of product and began to transport the pallet toward the shipping dock area using a pallet jack when they collided with the forks of a stationary forklift, resulting in a lower right leg fracture.
