Final narrative
A deck engineer was using an endless loop rope to moor a tender vessel to a dredge. The rope was secured to the port side cleat at the stern of the vessel. The dredge was in its cut, moving away from the vessel. As the engineer brought the rope over the cleat, a finger on his left hand was caught between the cleat and the inside of the rope. The fingernail and flesh from the first digit were amputated.
An employee was assisting with loading product (sacks of dirt) onto a barge. The sack of dirt swung and pinched the employee between it and other sets of product on the barge. The employee suffered a neck compression injury and was knocked unconscious.
An employee was on a barge adjacent to a lift that was being tested. As the load was lowered, a nylon stiffener broke free from existing structural steel and fell 80 feet, striking the employee in the right forearm. The employee sustained two broken bones in the forearm.

During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.
An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.
