An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

On May 16, 2025, a parking attendant was directing traffic at a parking lot exit after a concert. A taxi ran over her right foot, which caused her to fall onto her left side. She was hospitalized with fractures to her right foot, left hip, and left arm.

On April 25, 2024, an employee was assisting with moving a boom lift when his ankle was run over by the lift. The employee sustained a broken right ankle and was hospitalized.

An employee was doing inventory (counting boxes) while standing on the second rung from the top of an A-frame ladder. He lost balance and fell backward to the concrete floor, hitting his back and head on the way down. The employee was hospitalized with a skull fracture.

An employee was in a church repairing an air conditioning unit in the ceiling when he fell 12 feet to the floor and sustained a broken left wrist. He was hospitalized and required surgery.

At 11:00 a.m. on October 4, 2022, an employee was setting up a camera at a football stadium. He fell off the camera platform and landed on a row of seating. He suffered four broken ribs and was hospitalized.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
