Final narrative
An employee was operating a 12-inch dual compound miter saw to make a 45 degree angle cut into a 1-inch thick lead ingot. The employee wore leather-welding gloves and had his left hand on the ingot to hold it in position. As the saw blade contacted the ingot, the ingot shifted, which caused the employee's left hand to contact the saw blade. The employee sustained a laceration to his left hand.
An employee was abrasive blasting in the fan room on a ship. He backed into a 12-inch dust collector chute. The suction pulled the employee's left shoulder into the chute and the employee lost control of the blasting hose. Blast media caused an avulsion to the employee's upper right bicep.
An employee was in the basket of an aerial lift when blast grit from a co-worker's hose struck the employee's right upper arm, resulting in a severe laceration.
The injured employee was passing lead bricks so they could be cut to size while another employee was picking up the bricks from a pallet. Two of the bricks were stuck together by grease. The other employee forcefully dropped the lead bricks on the pallet to free them.
