On January 16, 2019, an employee was bending sheet metal on a hydraulic press brake. The height adjustment guard had loosened and created a larger clearance between the sheet metal and the die. The press brake was activated via the foot pedal while the employee's fingers were in the point of operation. His left index finger was crushed, resulting in a partial amputation.
