An employee was in a wood shop, using a table-mounted radial arm saw to cut wood for pallets. As he turned after deactivating the saw, the saw blade cut his finger, causing an amputation. The part of the blade that cut him was not guarded at the time.

On January 10, 2019, an employee was helping in the operation of a sheet metal slitting machine and was operating a sheet metal re-coiling machine. He was trying to tighten up the sheet metal coil before banding it. As the coil turned, his right pinky finger was caught in a pinch point between the coil and the tensioning bar of the re-coiler. The finger was cut about halfway through and was later surgically amputated just below the first knuckle.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
