A piece of plumbing was being removed from a piece of equipment (blender) between the discharge pump and the tub. The piece of plumbing slipped out of the clamp and fell onto an employee's right arm, fracturing it.

An employee was delivering a load to a customer location. As the employee made a right-hand turn onto a road, the tractor trailer tracked off the roadway and overturned. The employee sustained bleeding on the brain, a fractured collarbone, and a fractured finger.
Two employees were completing a post-job inspection of a centrifuge. One employee rotated the centrifuge rotating assembly to test if it could be turned by hand. The injured employee went to stop the rotating assembly when their left little finger was caught between the belt and pulley system. The employee sustained an open, non-displaced distal phalanx fracture and a fingertip amputation to the left little finger.
An employee was inspecting and writing down the serial number of a casing adapter tool in the yard. The employee rolled the tool over and the tool proceeded to roll over again, striking and amputating the employee's right index finger.
