An employee was attempting to free a stuck casting from the casting feeder hopper door. The employee sprayed lubricant on the casting and the casting freed from the door. The employee's right forearm was inside the casting feeder hopper door when the casting was freed and the door closed on and crushed his forearm.

ESCO Group, LLC.

An employee was using a hand-held angle grinder to cut 1/8" wire. As he was cutting the wire it became entangled in the grinding wheel and the wire struck the employee. The employee sustained lacerations to the first and third fingers on the left hand and two tendons were cut on the left index finger.

N. S. Giles Foundations, Inc.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

Weld Mech, Inc.

An employee was standing on the rig floor next to a polishing unit. His hand was placed on the polishing unit when the pump was lowered, resulting in amputation of their right thumb, ring, and middle fingertips.

An employee was using air to blow out the air wash to the blender and grinder station. As the employee reached to verify that the air wash was clean, the unit pinched and amputated their fingertip.

An employee was retrieving a lost drill pipe with a lifting bail when their left thumb got pinched between the table and handle of the lifting bail while trying to re-thread the pipe to lift it out. The employee suffered an amputation to the left thumb.

An employee was removing a core chuck from a stationary roll when their right ring finger was pinched between the chuck and the roll. The employee's fingertip was amputated.

An employee was using a foot-pedal-operated shear press. While the employee was adjusting material in the press, it activated and crushed their left ring fingertip, resulting in an amputation.

An employee was tightening a bolt on a brass plate located on top of a die when the ram block underneath the press contacted their right index finger, resulting in a partial amputation. The ram block was not locked out at the time of the incident.

An employee was loading a steel bar into a thread-rolling machine. The employee attempted to reposition the bar when it started spinning and caught his left glove. The employee's thumb was pulled into the feed table roller, resulting in fractures to the left thumb, wrist, and forearm.

A commercial jet aircraft crashed into the northwest corner of the facility and a fire resulted. An employee inside the facility sustained burns to his head, neck, and back.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Owl's Head Alloys West Point

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
