An employee was changing a tire on a forklift. The tire exploded injuring the employee's right hand and resulting in the medical amputation of the right index finger.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was using a clamshell tilt table to maneuver a tire when their left ring finger was pinched by the table and later amputated. The employee was hospitalized.

On November 11, 2024, at approximately 10:45 a.m., an employee was using compressed air to blow out the area under the buffer machine. Their right arm was caught in the rotating spindle, resulting in a laceration to the upper arm. The employee was hospitalized.

An employee was working at the trim station of a precured tread finishing line. While removing trim waste material from the tension roller beneath the line's conveyor, the employee was caught in a pinch point, resulting in a fracture to their right ulna.

An employee was walking in the warehouse when they were struck by a forklift. The employee was hospitalized with a fractured left hip.

An employee was putting air in a commercial roller tire when the lock ring blew off, and the rim struck him in the head. He sustained head and internal injuries.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
