Employees were removing a single-phase, dead-end utility pole. The pole was lifted out of the hole and the boom of the truck was rotated to lay the pole down with the top toward the front of the truck. The pole guides were opened and the pole top shifted in the opposite direction. The top of the pole contacted the phase approximately 25 feet from the old dead-end insulator and an electrical arc occurred. The truck became energized with up to 7,000-volts. The injured employee was holding the bottom of the pole down. He was pushed into the step-opening of the truck and then fell to the ground. The employee sustained second-degree burns on his right shoulder and third-degree burns to the back of his legs.

An employee received an electric shock while connecting a new construction residential home to an outside pedestal.

An employee was standing on the ground, pulling overhead cable using fish tape. The fish tape broke or detached from the cable, causing the employee to fall to his knees. He tore quadriceps tendons in both knees.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

Employees were pulling three-phase power lines to a power pole. An employee's hand was caught between a pulley and a rope, resulting in amputation of the employee's index, middle, and ring fingertips.

An employee was cutting through two secondary wires with a battery-powered cutting tool. The wires were energized at 120/240 volts; when the employee cut through the insulation around a wire, an arc flash burned the employee's left thumb.

The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
