At 4:30 a.m. on June 20, 2025, an employee suffered a gunshot wound during a verbal altercation in a parking lot. The employee was hospitalized.

An employee was placing a heavy piece of metal onto a cart when the cart tipped over and the metal piece slipped, lacerating the employee's right middle finger and resulting in a fingertip amputation.

An employee was descending the ladder on a 48-inch platform and fell to the ground, sustaining a broken ankle.

An employee was performing maintenance on an extendable conveyor and lost his footing while standing under the running conveyor. His left arm was caught between the drive roller and snubber rollers in the conveyor and amputated.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee was checking the function of newly installed actuators when they were struck by a pressurized release pipe. The employee suffered fractures to the orbital socket and shoulder, requiring hospitalization.

A ramp in the trunk of a delivery van struck an employee's calf. She suffered a broken leg and was hospitalized.

An employee was completing a service call and looking at a water pump when the pump cover plate struck their forehead. The employee sustained a forehead laceration and fracture requiring hospitalization.

An employee was removing chains from a steel tube. A sorting hook attachment was caught on the staging area cross member. When he raised the chains, the sorting hook snapped back and struck the employee's face causing a fractured jaw.

An employee was in the sawmill basement using a push broom and shovel to clean wood chips and sawdust. The employee's right arm was struck by a wooden board between the wrist and elbow, resulting in a fracture.

An employee's left bicep was lacerated by high-pressure water during water blasting operations. The employee was hospitalized.

An employee was performing flagging operations on a closed road. A postal vehicle was allowed onto the road to make a delivery. The employee was struck by the postal vehicle as it was backing up, resulting in a broken leg.

An employee was walking and taking post-locate photos. He stepped on a ceramic drainage tile and his foot/leg punched through the tile. His leg was lacerated by the broken tile. The employee was hospitalized with internal bleeding due to a punctured femoral artery.

Employees were preparing to place a section of metal grating measuring approximately 50" x 18" and weighing 40 to 50 lbs. The injured employee grabbed the grating to pull it toward himself and another employee was moving to the other side to assist with the lift. As the injured employee pulled, his right foot slipped and twisted. He was hospitalized with a fractured right leg.

An employee was in their personal vehicle awaiting instructions from the construction contractor on posting traffic cones. The driver of a motor vehicle swerved to avoid a steel plate in the roadway and struck the employee's car. The employee was hospitalized with four fractured ribs.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
