An employee was manipulating a finished part by hand by sliding it across the work table when the metal part fell over, catching his right ring finger underneath it and resulting in a fingertip amputation.

An employee was at a customer site, helping align a belt on a machine, when his fingers were caught between the belt and a sprocket. He suffered fractures and avulsions, and the ring fingertip on his left hand was medically amputated.
An employee was pulling a pallet jack backward when she lost her balance and fell backward, striking her head on the floor and suffering a fracture at the base of the skull.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface.
