An employee stepped into the back of an empty trailer that was parked at the loading dock. The vehicle drove away from the dock and the employee fell to the pavement, sustaining fractures to their right tibia and fibula.

A driver pulled a truck away from the loading dock and then climbed up on the bumper to pull down the roll-up door. The driver slipped from the bumper and fell approximately 4 feet, striking his head resulting in a skull fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.
