An employee was case picking when she tripped over a piece of wood sticking out of a pallet on the floor. She fell to the concrete floor, suffering broken vertebrae.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was assisting with maintenance and servicing a conveyor that was making an unusual noise. The employee's right arm got caught in the conveyor between a belt and a roller. Their right hand and arm were crushed, resulting in amputation of the arm.
