A crew was performing routine rod work. While making up a rod triple, the rod triple slipped out and made contact with an employee's left arm. The employee sustained a fractured elbow and forearm.
On the workover rig floor, crew members were lining up the Kelly valve to a joint of tubing utilizing the winch line. An employee's right thumb was pinched between the winch line and the Kelly valve. The thumb tip was amputated past the nailbed.
