Final narrative
An employee was installing a new AC motor on a sheller. The come-along chain was pulled into the conveyor below and pulled the chain down onto the employee's left hand, amputating the middle and index fingers.
An employee was unclogging a bran airlock system. After removing a pipe spool piece and removing bran from inside the airlock, the employee turned the machine on to see if it was unlogged. The machine then crushed the employee's left hand.
