An employee was operating a press. A billet was uneven as it entered the loader; it hit the ram and then sheared off a hydraulic line, which sprayed hydraulic fluid into the employee's mouth and was ingested. The fluid was ignited, and the fireball caught the employee's hair on fire and he sustained first-degree burns to his forehead and hand. Ingestion of the fluid and inhalation of the fumes affected the employee's airway. He was hospitalized.

An employee was walking down an aisle pushing a cart when five 900-pound bundles fell. Two bundles struck the employee causing a laceration to the back of the head, three broken ribs, a broken foot, and four broken toes. The employee was hospitalized.

On April 23, 2023, at approximately 7:30 a.m., an employee was cleaning a cast trough when he was sprayed with molten aluminum. The employee was hospitalized for burns.

An employee was rolling an aluminum log on the log table/platform when their left hand was caught between the log and the platform, amputating the pinky fingertip.

An employee was using a measuring tape to measure an aluminum log on a conveyor. The conveyor started while the employee's hand was between two of the logs, crushing his fingers and resulting in the surgical amputation of his left middle fingertip.

On November 27, 2023, an employee was emptying a 2-inch product line that transports sodium hydroxide liquid from a rail car to a 275-gallon tote tank. The nozzle came out of the tote and sprayed sodium hydroxide onto the employee's face. The employee was hospitalized with chemical burns to their face, mouth, and neck.

An employee was working with sulfuric acid as part of the production process. While transferring the chemical from a large container to a smaller container, it splashed on his body and hand, resulting in a chemical burn.

An employee knelt in wet concrete while performing work as a concrete finisher and sustained a chemical burn to the right shin. The employee was hospitalized and required surgery.

An employee was using a 5-gallon bucket to unload acid product from a tank. Residual product leaked into the containment area, causing the employee to sustain first- and second-degree burns to the chest, as well as third-degree burns to the arms.

An employee was transferring an alkaline cleaning chemical from a bulk container into 1-gallon containers. The employee lifted a gallon container by its label tag. The tag broke causing the container to fall approximately 14-18 inches. The container struck the ground and the contents splashed onto the employee causing chemical burns to their eyes.

An employee's left arm was caught in the fingers or jaws of the head stretcher machine, resulting in a laceration. The employee was hospitalized.

An employee was performing regular duties in the cast house and sustained heat exhaustion.

At 7:01 p.m. on June 16, 2025, two employees were pushing a saw horse with a bundle of aluminum on it when it flipped over onto the injured employee. The employee was hospitalized with a broken leg and required surgery.

On June 4, 2025, at approximately 1:00 AM, an employee was working when they experienced cramping and were hospitalized with a heat illness.

An employee was performing maintenance on a press puller and aligning the puller when it fell. The puller electrical panel struck their left upper back, resulting in fractures to their T12 and L1 vertebrae. The employee was hospitalized.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
