Final narrative
An employee had his left hand on the concrete floor while installing a vapor barrier on exterior walls. A conduit pipe hanging above fell out of its fitting approximately 12" and hit his left index finger, cutting off approximately 1/2" - 3/4" of the tip.
An employee was trimming cured spray-foam insulation on a wall while on a step ladder. The employee lost balance and fell to the ground. They sustained a laceration to the head and fractures to their left shoulder and a rib.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
