Two employees were replacing cartridge filters at the recycle oil filter in a hydrocracking unit. While removing compression springs from the top of the filters located within the filter pot, vapors that had accumulated within the filter tubes ignited, causing a flash fire. Both employees suffered burns to multiple body parts and were hospitalized.

An employee was inspecting a furnace. While removing an access door from the furnace, the employee's finger was pinched between the access door and the outer furnace beam support, resulting in amputation of the left little fingertip.

An employee was loading merchandise onto rolling containers that were on a remote-operated order picker. While reorganizing merchandise to prevent top loading, the employee's wireless control device contacted a container and caused the order picker to move forward. The employee's right ring finger was caught between two containers and was amputated above the first knuckle.

The injured employee was operating an elevator at a construction site. He went from the tenth floor to the eleventh floor with a plumber and a scissor lift in the elevator. The plumber was using a remote to operate the scissor lift. As the scissor lift was exiting the elevator, a pipe that was on the scissor lift struck the injured employee's chest. The employee was hospitalized.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.
