Final narrative
On May 13, 2019, an employee was asked to locate a motor in the storage room. About 20-30 minutes later a maintenance employee came into the storage room and found the injured employee leaning against a rack and bleeding. A ladder was found on the floor and the injured employee later said he had been on the ladder about 4 feet high. The employee was hospitalized with a skull fracture.
An employee was monitoring a filling tower auger when it became clogged with a cheese curd. The employee shut off the clogged auger and reached in the tower to clear the blockage. The rotating auger caused amputations to their right index and middle fingers and partial amputation of the right ring finger as well as other injuries to their right hand. The machine was not locked out/tagged out at the time.
