An employee lost control of his standup lift vehicle and dismounted it. The steering tire rolled onto his left foot, de-gloving his foot from the metatarsal to the toes and resulting in the loss of three toes.

An employee was driving a double pallet jack down an aisle when another piece of equipment came through the aisle. Realizing that the two pieces of equipment were about to collide, the employee jumped off the pallet jack. It then ran over his leg, breaking his leg/ankle.

An employee was operating a forklift when the load shifted. The employee jumped from the forklift and it rolled backward, falling off the ramp and pinning the employee underneath it. They sustained head trauma and injuries to the shoulder, arm, pelvis, and leg.

An employee was operating a rider-style powered pallet jack. He lost his balance causing him to fall off and become trapped between it and a wall. He suffered a broken pelvis and was hospitalized.

An employee was operating a stand-on mower down a hill when the mower started sliding. The employee jumped off the mower and fell to the ground. Their hand was cut by the mower blade, resulting in a laceration and a thumb amputation.

An employee was driving a package car around a building on UPS property when the vehicle struck a bump. The force from hitting the bump unbuckled the employee's seatbelt and caused them to fall out of the vehicle onto the asphalt. The employee was hospitalized and required surgery for fractures to their hip and pelvis.

On April 14, 2025, an employee was fixing a belt on a mattress tape machine. The belt had folded over; as he was unfolding it, it caught his right ring finger and pulled it into a roller. He suffered a partial amputation through the finger bone.

An employee was stepping down from an order picker when they tripped over the forks of another electric pallet jack that was parked nearby and fell to the ground. The employee sustained a displaced pilon fracture to their left tibia.

An employee was using a rotary saw and lacerated their left index and middle fingers, resulting in an amputation that required hospitalization.

On January 11, 2023, an employee was operating a saw machine. He stepped onto the cutting table and fell backward approximately 20 inches onto the floor. He struck his head and neck on the metal frame of the equipment and was hospitalized.

The injured employee had been using a powered industrial truck to unload a delivery of foam buns coming into the plant. He went to assist the operator of a carousel saw in getting a foam bun to go through the saw. The injured employee got on top of the foam bun which was on the saw table and his hands and knees depressed into the bun. His left kneecap was lacerated by the saw blade as the bun went through the saw. The employee was hospitalized with tendon damage that required surgery.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
