Final narrative
Two employees were trying to move a 62-foot-long, 2,000-pound part over on a conveyor table. On the third attempt, the part fell off the conveyor and landed on one employee's left foot. The employee suffered two broken toes and was hospitalized; the little toe was medically amputated.
An employee was tightening a bolt on a brass plate located on top of a die when the ram block underneath the press contacted their right index finger, resulting in a partial amputation. The ram block was not locked out at the time of the incident.
