An employee was removing a gear from an engine using a crane. The employee was unhooking a come-a-long when the crane operator noticed tension on the gear box. The crane operator lowered the gear and the employee unhooked the come along. The gear box came loose, swung, and hit the employee, pinning him between the gear box and bulk head. He fractured his left arm and ribs on the left side.

A temporary employee was tasked with pressure washing ships. The employee went to connect the pressure washer to an electrical terminal and received an electric shock. He experienced muscle cramps in his legs.

A temporary employee was standing on scaffolding removing oxidation with a torch when they fell off and landed on an anvil on the ground, resulting in fractures to their L1 -L5 vertebrae and a left kidney laceration.

At about 8:00 a.m. on April 5, 2022, an employee was using lifting equipment to lift a pump that was to be repaired. The pump shifted and the employee's right hand was caught between it and a boat deck. The employee's right index and middle fingers were crushed and the employee was hospitalized.

InterCon Construction, Inc.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. The employee suffered head trauma that required hospitalization.

RAVA Construction, LLC

An employee and a co-worker were performing a pick inside a clear well. They rigged the skid pan that was full of broken concrete. When lifting, the load began to swing toward a wall. The employee tried to stop the skid pan from swinging and was struck by the pan, resulting in fractures to their left hip and wrist.

The employee had just completed refueling a lattice crane that was breaking up material and was winding the fuel hose back into the fuel truck when he was struck by the catwalk/stairs of the crane. The employee was hospitalized with a laceration to his backside, possible internal bleeding, and a broken hip.

An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

An employee was lifting four bags of a lime blend weighing approximately 10,000 pounds using a crane. The load swung and pinned the employee against a structural I-beam, resulting in fractures to the hip and pelvis.

A temporary employee fell from an A-frame ladder to the floor about 5 to 6 feet below. He suffered fractures to his left hip and was hospitalized, requiring surgery.

A temporary employee was operating an electric pallet jack at a loading dock. While maneuvering the pallet jack into a trailer, the equipment loaded onto the pallet jack began shifting to the left. The employee's left foot was caught between a yellow guardrail and the side platform of the pallet jack, and his left ankle was crushed and fractured.

A temporary employee was cleaning a room and compressing the contents of a garbage bin using her right foot. As she pulled her leg out of the bin, she lost balance and fell to the ground, landing on her left hip. The employee sustained a fracture to her left hip and was hospitalized.

A temporary employee was filling a sweeper with gas at a construction site. He was shocked by the jumper cables, which caused him to move his right hand into the sweeper's rotating fan blades. His middle fingertip was amputated.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.
