On June 7, 2019, an employee was changing a gasket on the CSP line. The employee opened a clamp and was sprayed by hot water, suffering second degree burns to both arms, the chest, the back, and the neck.

On August 22, 2018, an employee of Industrial Machining Services was replacing a valve on the tank cluster in an isolated work area. After the employee finished welding on the tank outlet cluster, the employee began to assemble the valve when an unknown hot water line sprayed the employee in the arm, resulting in second and third burns and a hospitalization.

An employee connected a steam line to a hose to clean equipment when the fitting broke loose. They were struck by steam in the left inner thigh, resulting in burns that required hospitalization.

An employee had turned off the ball valve on a waterpipe system and was removing the plug when the coupling system attached to the strainer came apart. Hot water sprayed on his arm and back, resulting in first- and second-degree burns that required surgery.

Oklahoma Steel & Wire Co., LLC

An employee was using a shovel to remove waste vermiculite from molten zinc. The metal had been placed in a bin and partially hardened. The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

Blue Bell Creameries, L. P.

An employee was using an end mill to mill out a section of a metal door frame that he was fabricating. Metal shavings built up and he could not see the marking to cut, so he used a small brush to remove the shavings. While brushing off the shavings, his glove became caught in the spinning mill bit and pulled his hand in. The employee's right index finger contacted the spinning bit, resulting in a partial amputation to the side of the finger.

DFA Dairy Brands Ice Cream, LLC

An employee was unjamming cookie dough from a machine when the rotating pump caught her left index and ring fingers, resulting in fingertip amputations.

The Ziegenfelder Company, Inc.

An employee was marking a malfunctioning mold when their finger got caught between one section of rollers on the mold and the guarding on the next section of rollers. The employee's finger was amputated below the nailbed.

An employee was palletizing ice cream when his fingers got caught in a chain and sprocket, resulting in amputations of the middle and ring fingers.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
