An employee was making a grease head connection. He pulled on the sling connected by a crane to balance the grease head. He was stabilizing the 5-1/2 lubricator with his left hand when the lubricator joint slipped into a tight space causing the 4-3/4 grease head to crush his left index finger against the lubricator. The employee's left index fingertip was amputated.

An employee picked up one end of a 300-pound motor, stepped back to put it on a stand, and slipped on ice. As he fell backward, the motor struck him mid-chest. His sternum was fractured in two places and four of his ribs were broken.

The injured employee and a coworker removed a valve body to unclog pods in an auger. After they unclogged the pods and reloaded them, they proceeded to reinstall the valve body. As they were picking up the valve body, the coworker slipped and the valve fell onto the injured employee's left pinky finger, resulting in a fingertip amputation.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.

An employee was using a torch to loosen a bolt on a water trailer. Natural gas was present in the water and it ignited, causing a fire. The employee sustained third-degree burns to their upper leg.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
