Final narrative
On June 24, 2019, at 1:20 p.m., an employee and coworker were attempting to install a transverse compartment box weighing approximately 60 pounds in the back of a cab when the box slipped out of the employee's hand. The employee's left index finger was then pinched between the box and the wall of the cab, resulting in a fingertip amputation.
An employee was going on break in the company break area. The employee reached in to move the chair underneath them as they sat down and their right index finger was pinched between the pan of the chair and the frame. The employee's finger was partially amputated.
An employee was placing transport wheels to move a pump housing when their left ring finger got caught between the pump housing and a transfer wheel, resulting in a fingertip amputation.
An employee was positioning a stand-up lift truck to pick up parts. When the operator turned the steering wheel, a tire on the lift grabbed the floor. The lift swung around, causing the operator to fall out of the lift. The operator fell up against racking and the lift pinned him between the rack and the lift fracturing ribs.
An employee was walking on a dock plate to open the door for the next trailer. He tripped on a cargo strap that was laying on the dock and fell. The employee sustained a fractured right hip, requiring hospitalization and surgery.
On September 18, 2025, an employee was rolling a stack of two steel racks containing automobile bumpers when the top rack dropped, pinching their right little finger. The employee suffered a partial amputation.
An employee was exiting an ambulance when his foot slipped, causing him to hit either the portable aluminum step or the concrete floor. He suffered a broken left elbow.
Two employees were sliding a 200-pound steel mounting plate off a worktable onto the forks of a forklift. They slid the plate until it was half on the table and half on the forklift, and were making an adjustment to get the plate fully on the forklift when the plate pinched one employee's left ring finger. He suffered a partial amputation to the fingertip.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.
An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
