On August 8, 2022, an employee was working on an automatic palletizer. A pallet got stuck on the roller; as the employee tried to free it with a pry bar, his right middle finger was caught between the pry bar and the roller. The fingertip was amputated.

On December 12, 2020, an employee tripped over a cement block and fell onto a door manifold that was sticking up about two inches. The employee was hospitalized with a ruptured spleen.

An employee was cutting a piece of metal with a saw when the saw blade broke and lacerated his leg, requiring stitches.

A maintenance employee was hospitalized after a brake on a horizontal grain conveyor ruptured and projected a metal shard into his left upper leg.

An employee was walking to a customer's door to deliver a package when they tripped on uneven pavement and fell to the ground. The employee sustained a hip fracture.

An employee was leaving work for the day when he tripped over mulch and fell to the ground. He was hospitalized with a fractured hip.

An employee was walking on the sidewalk toward the front entrance of the building to start her work shift. She tripped over uneven pavement and fell onto the concrete sidewalk. The employee was hospitalized with a fractured left hip and a sprained left knee.

An employee was walking into work through a doorway when she tripped on the door sill and fell to the floor, fracturing her humerus.

An employee was leaving for lunch. The wheel of her knee scooter struck a divot in the floor which caused her to fall into a metal rack. The employee sustained a broken right shoulder.

An employee was preparing to unjam an auger shaft while standing on a forklift-elevated, job-made platform. The employee's wrench slipped off the equipment, causing them to lose their balance and fall. The employee landed on the ground 14 feet below and suffered fractures to the orbital bone, left elbow, and lower back.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.

An employee had been cleaning a grain bin. He walked across the bin and his left foot contacted an unguarded floor auger, resulting in severe lacerations to the foot. The auger was not locked out/tagged out at the time.

On September 23, 2025, an employee was monitoring a feed roller when they tripped and their right hand contacted the roller. The employee was hospitalized and their right hand was amputated at the wrist.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
