An employee backed up a tugger to attach it to a transport rack. After the employee dismounted the tugger, his fingers were caught between the tugger and a part on the transport rack. He sustained the amputation of the right middle fingertip and a contusion to the ring finger.

An employee was operating the hydraulic press brake. The pull back safety device malfunctioned and the employee's right thumb was caught in the machinery resulting in a thumb tip amputation.

An employee entered the back of a semi-trainer to prep for loading. There were approximately five 8' x 10' x 1"-thick sheets of plywood used as dunnage on the previous load that were left on the trailer and needed to be removed. They were stacked on end and secured to the wall of the trailer. The employee untied them in order to move them. The dunnage fell on and injured the employee.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

ACCO Brands Corporation

An employee was troubleshooting a tabletop binding machine at a customer location when the machine cycled and contacted their left middle finger, resulting in an amputation without bone loss.

An employee was setting up a CNC bender to fabricate a new product prototype. A piece of tubing was drawn back into the bending machine and pulled the employee's right hand in with it. His fingertip was caught between the bending and damping dies and amputated.

At 10:15 a.m. on May 17, 2022, an employee was cutting a sheet of half-inch oriented strand board on a table saw when the saw blade contacted the employee's right hand. The employee's index, middle, and ring fingertips were amputated.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
