Final narrative
Two employees were installing a sink. An employee was underneath the sink while the injured employee was pushing the sink down. The sink fell through and broke. The injured employee's right arm went through the sink and his forearm was lacerated.
An employee was using a forklift to deliver rebar to a jobsite. The rebar could not fit through the delivery gate and was raised in order to clear the gate.
