The employee was walking on the platform towards the exit side were the ladder was located. The employee tripped on the corner high beam and he reached out to support himself with the crane cable that was moving. His four left hand fingers were amputated by the crane cable and pulley.

An employee was fastening a tug boat to a barge when his fingers were crushed between the boat knee and the barge. His left index and middle fingertips were amputated.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.
