An employee was putting together an order by stacking materials on wood pallets. The employee went to step over a pallet, caught his foot, and fell to the concrete ground. He landed on his shoulder and was hospitalized for shoulder surgery.

The injured employee was assisting a forklift operator in the preparation a load of solid core doors for a customer. The injured employee was standing on the platform where the doors were placed for transport and the forklift operator began to move the load. The load shifted and the employee was struck and caught between the solid core doors. The employee sustained a fractured rib and pneumothorax.
