Two employees were placing cable while working from a line truck with an attached reel carrier. The injured employee was observing the reel as it was taking in pulling tape when the reel bar came around and smashed his left hand between the lifter and the bar. His thumb was injured and required surgery.

An employee was dismantling a four-tier scaffolding system. When the employee stepped on a wooden scaffold plank, it failed and the employee fell to the cement floor about 28 feet below. The employee was hospitalized for broken bones and body and head trauma.

A technician was ascending an extension ladder that was placed on a strand behind a residence. The ladder began to slide on the strand and the technician fell approximately 4 feet. The technician landed on their feet on sloped ground and sustained broken bones in both ankles.

An employee was starting up a grinder pulverizer when he noticed that water was leaking from the pulverizer. As he tried to wipe away the water, his fingers became trapped by one of the pulverizer fins. He suffered a hand injury and was hospitalized. The pulverizer was running at the time of the incident.
